Osarizawaite is a rare secondary mineral found in the zone of oxidized over lead-bearing deposits.

RUSK COUNTY: Osarizawaite occurs in the oxidized zone at the Flambeau Mine, near Ladysmith (Jones, Jones, and LaBerge, 1999).
